## Documentation

## What Ca Medical Exclusions does

The Ca Medical Exclusions MCP server provides access to California's Department of Health Care Services (DHCS) Medi-Cal Suspended and Ineligible (S&I) Provider List. It is intended for screening providers that may be barred from California's Medicaid program.

The primary check accepts an optional provider name, NPI, license number, and result limit. Results can include the provider's name, AKA or DBA, address, provider type, license number, provider numbers, suspension date, active period, and matching information. The dataset contains approximately 23,400 rows at the documented capture point.

Identifier matches should be treated differently from name matches. An NPI or license match is an identification based on a provider-specific value. A name match is only a candidate lead, even when the spelling is exact, because the source does not include dates of birth and some entity records lack first or middle names.

## How it works

The Ca Medical Exclusions MCP server uses the California Health and Human Services Open Data Portal's CKAN datastore as its source. The datastore provides the full list as JSON through a stable resource ID, while a separate resource metadata endpoint supplies the source's `last_modified` value. Responses expose that date as `data_as_of`, allowing callers to assess the age of the indexed copy.

The deployed pack serves a baked index rather than querying the upstream dataset for every request. The index is generated from the full source list and injected into calls at deployment. Because DHCS and CHHS update the list monthly, maintainers need to regenerate and redeploy the index to refresh the data.

NPI matching extracts 10-digit tokens from the source's combined provider-number field. License matching uses the separate license field, compares digits only, and removes leading zeroes. Name matching is case- and punctuation-insensitive and checks the assembled provider name along with AKA or DBA values.

The coverage tool reports the number of exclusions, identifier coverage, provider-type diversity, the oldest and newest suspension dates, the list-wide statutory basis, and the source metadata date. The statutory basis applies to the list as a whole; the source does not provide a reason, statute, or case citation for each individual row.

## Setup and configuration

The Ca Medical Exclusions MCP server can run as a local stdio package with `npx`:

```json
{
  "mcpServers": {
    "ca-medical-exclusions": {
      "command": "npx",
      "args": ["-y", "@pipeworx/mcp-ca-medical-exclusions"]
    }
  }
}
```

It can also be connected through the scoped gateway endpoint:

```json
{
  "mcpServers": {
    "ca-medical-exclusions": {
      "url": "https://gateway.pipeworx.io/ca-medical-exclusions/mcp"
    }
  }
}
```

The integration is keyless. The local package exposes only this pack's tools. The gateway endpoint also lists shared Pipeworx meta-tools, including routing and discovery tools, while the full gateway endpoint provides access to the wider catalog. HTTP callers can invoke tools through the documented `/v1/tools/` endpoint.

## Tools and capabilities

- `ca_medical_check_exclusion`: screen by name, NPI, or license, with an optional result limit.
- `ca_medical_exclusion_coverage`: inspect record totals, identifier availability, provider types, suspension-date range, statutory basis, and source freshness.
- Name searches include AKA and DBA values.
- Provider numbers can contain NPIs and non-NPI identifiers; only 10-digit values are treated as NPIs.
- Responses include provider-type information and distinguish list-wide statutory context from per-record data.
